Staff and Care Providers Survey LTC COVID-19 Review

Thank you for agreeing to fill out this survey about COVID-19 in long-term care homes. Your answers are important to the Long-Term Care COVID-19 Review Panel which is reviewing the response to the COVID-19 pandemic.

In its report to the Government of PEI, the Panel will share its findings on what worked well, where there are areas for improvement and make recommendations for change.

You don’t have to answer any question you do not want to but it will help us if you can fill in as many as you can. Your responses are anonymous, meaning that we will not report findings that reveal the identity of any survey participant.

Personal information from this survey is collected under section 31(c) of Prince Edward Island’s Freedom of Information & Protection of Privacy Act as it relates to long-term care services.
